Welcome to the Sunnyslope "VIKINGS" Sports Program.  If you are interested in playing a sport please pick up an Athletic packet from the Athletic or Front Office.  To be cleared you must have the following:

1. Recent Physical Evaluation completed and performed by a medical doctor, on or after March 1st of current school year.  (Required yearly)
2. 2 sided yellow Emergency Consent Cards & Health Insurance Info., including policy#. (4 sections)  If no insurance, school insurance applications are available and must be returned with payment.  (Required yearly)
3. State law and AIA bylaws MANDATORY "Concussion Statement & Acknowledgement Form" signed by both student and parent.  (Required once)
4. State law and AIA bylaws MANDATORY "Certification of Completion" for the on line Concussion class.  Go to aiaacademy.org/brainbook and register (top right), once student passes the course (they can repeat if needed), they print a certificate and turn it into the Athletic Office (Required once)
5. Sports at Risk Video Acknowledgement Form - signed by student once viewing of video. Shown by PE teacher or in the Media Center Tu., & Th., @ 2:45p sharp (Required once)   
6. Copy of birth certificate (Required once)

FAMILY ATHLETIC PASS

We highly encourage parents to purchase a Family Athletic Pass for ONLY $60.00, which allows parents and siblings under high school age into all REGULAR SEASON HOME GAMES. 
Applications can be picked up from the bookstore.
